CHHA Job Description Job Title: Certified Home Health Aide Position Overview:To provide care as assigned by Open Systems Healthcare, Inc. through the Plan of Care in the client home, facility, and/ or community. Providing quality, safe and effective home care services while adhering to the independent living philosophy of the client, in conjunction with OSH's philosophy and mission statement. Requirements: - High School diploma or GED equivalency, where applicable.
- CHHA certification that meets State and OSH standards through the NJ BON and Division of Consumer Affairs
- Evidence of Tuberculosis screening that meets CDC guidelines.
- Passed OSH competency exam with a score of 80% or greater.
- Ability to perform tasks involving physical activity, which may include heavy lifting and extensive bending and standing.
- Able to work in a stressful work environment.
- Must have a reliable form of transportation.
- Adequate communication skills.
- Recognizing emergencies and knowledge of emergency procedures.
- Maintenance of clean, healthy and safe environment.
- The physical, emotional and developmental needs of and ways to work with populations served by OSH including, the need for respect for the client/consumer and his or her privacy.
- Appropriate and safe techniques in personal hygiene and grooming.
- Ability to provide ADL's as defined through the NJ BON.
Essential Job Functions: - Ensure quality and safe delivery of home care services.
- Participate in development and implementation of client plan of care in accordance to OSH policy and procedures.
- Provide home care services that reflect client plan of care.
- Communicate all relevant client information with OSH in a confidential and timely manner.
- Accurate & timely documentation of services provided.
- Comply with Federal, State and Local Laws.
- Comply with all OSH policy and procedure requirements.
