Applying for New York State Unemployment Insurance
Here are the things you need to do: |
1. You must complete the New York State Unemployment Insurance application. You can apply online at https://ui.labor. |
2. To complete the online application you will need: - Your Social Security Number - Valid - Your complete mailing address and zip code - Telephone number where you can be contacted between 8 am and 5 pm Monday through Friday - Complete name, address, zip code, phone number and wage information of your most recent employer and any employer you have had in the last 18 months - Alien Registration card number (if you are not a U.S. Citizen) - It is preferred that you provide, Employer Registration Number or Federal Employer Identification Number (FEIN) of your most recent employer (FEIN may be found on your W-2 forms) |
